Regulatory context

What OCR is citing, and what the proposed rule changes.

More than half of 2025 OCR enforcement actions cited the same deficiency: failure to conduct an adequate security risk assessment. The proposed Security Rule update published in January 2025 would eliminate the “addressable” category, require encryption and MFA everywhere ePHI exists, and tie annual risk analysis to a technology asset inventory.

Healthcare organizations also carry the operational reality of EHR platforms, connected medical devices, and vendors that touch patient data. We have secured all of it, and we built Timberline so healthcare teams could use AI without sending PHI to public model APIs.

  • Risk assessments structured for OCR review
  • Encryption and key management for ePHI at rest and in transit
  • Business associate security due diligence
  • 72-hour restoration planning for critical systems
